Patient Adherence to GLP-1s Linked to Out-of-Pocket Costs

A study analyzing 8,914 patients found that out-of-pocket costs significantly impact adherence to GLP-1 weight loss medications, with a notable increase in discontinuation when monthly costs exceed $75.

The study, published in JAMA Health Forum, examined medication compliance over 1.5 years across five cost tiers. Even at the lowest cost ($0-$21), three out of four patients stopped taking their GLP-1s within a year.

Adherence dropped significantly when monthly out-of-pocket costs surpassed $75, with nonadherence rising to over 80%. This tipping point was unexpected, as adherence remained similar across lower cost tiers.

Factors like side effects and insurance policies, including mid-year changes, prior authorizations, or step therapy, can also interrupt prescription continuation, though not directly measured in this study.

The lowest adherence rate, around 17%, was observed in patients paying $168 or more monthly. These patients were more likely to reside in Southern states and have high-deductible insurance plans.

Previous research suggests lower-income populations often opt for plans with high cost-sharing, meaning they may pay the full price of GLP-1s until a multi-thousand dollar deductible is met, leading to lower adherence.

While short-term financial barriers exist, future research may demonstrate long-term economic benefits of GLP-1s in managing chronic conditions like obesity, potentially reducing hospitalizations and healthcare costs.
